Every questionnaire, in one place
8 questionnaires in the patient app and 20 instruments on the doctors' site — all of them active, all in Persian and English. Here is what each one measures, who answers it and where its Persian text comes from.
In the patient app 8
The patient answers them whenever they like, under “Assessments”. The raw score and its trend over time are shown and kept in the encrypted backup.
| Questionnaire | Measures | Items |
|---|---|---|
| PHQ-9Patient Health Questionnaire-9 | Depression | 9 |
| GAD-7Generalized Anxiety Disorder-7 | Anxiety | 7 |
| BDI-IIBeck Depression Inventory, 2nd edition | Severity of depression | 21 |
| BAIBeck Anxiety Inventory | Symptoms of anxiety | 21 |
| RRSRuminative Responses Scale | Rumination | 22 |

| made by BivelaHypomania symptom checklist | Symptoms of hypomania, yes or no | 13 |
| made by BivelaMania symptom self-rating | Eleven areas of mania symptoms, as the patient sees them | 11 |
The last two are Bivela's own checklists, not the official MDQ and YMRS; their totals cannot be compared with those instruments. The translation, scoring and interpretation of the app's questionnaires are not clinically validated yet, so no interpretation is shown and the numbers are only for following a trend.
Doctors' site: full text 10
The doctor uses these at admission, weekly, at discharge or at follow-up. The questionnaires the patient answers are filled in on the doctor's phone or browser in “patient mode”; PHQ-9, PHQ-2, GAD-7, GAD-2, DASS-21 and ASRM can also be sent to the patient's own phone.

| made by BivelaBivela psychosis rating (adapted from the BPRS, 8 items) | Psychosis | Doctor | 8 | Bivela's translation of the official English; delusions as one item; a ready question on hallucinations and delusions |
| CGI-SClinical Global Impression: severity | Overall severity | Doctor | 1 | Bivela's translation of the official English |
| CGI-IClinical Global Impression: improvement | Improvement since treatment began | Doctor | 1 | Bivela's translation of the official English |
“Bivela's translation” means no official or validated Persian version was available, so the official English was translated in full and exactly. The source of each instrument's Persian text is stated on its form, in the library and in the report, and any doctor can check the text against its source part by part. PHQ-9's tenth question (difficulty at work and at home) is not scored.
Doctors' site: score entry 10
The text of these instruments is not in Bivela — its copyright belongs to its owners, or a valid Persian text has not reached us yet. The doctor uses their own licensed paper form and enters each item's score on the doctors' site by number and a short name; Bivela adds them up and applies the published criteria. These forms are never sent to the patient's phone.

Worrying answers
On the doctors' site, an answer that points to thoughts of self-harm (such as PHQ-9's ninth question or HAM-D's third item) is highlighted for the doctor at once and offers the suicide-risk screen. When a patient answers a questionnaire their doctor sent on their own phone and the answer is worrying, they are shown the way to urgent help.
